Wonderful is Form Tools! I used it to create application forms for new students to our school; it made wonders of collecting data. There is room to customize the theme of your choice to match the color scheme of your organization, though one has to take the extra mile to change the CSS. All in all, I can say big thumbs-up to the team.

I spent an entire week investigating contact form tools and services, only to be disappointed with each one, and dismayed to pay a monthly fee to use a script. Finding this tool was a blessing and saved me money and a ton of work. The fields and views were a little challenging to figure out, as was the mapping of the email field feature to enable a user autoresponder. Otherwise, the process was streamlined and easy to figure out. I found the answers I needed in the online forum.

I've been wandering everywhere looking for a form generator that could take over 100 submissions a month for free. Jotform let me down, even though their forms were amazingly good looking. This tool allows me to take a form from Jotform and use their database with it :)
